1-800-525-2225. You will be asked to provide the patient’s name, address, phone number, date of birth and insurance information. You will also be asked about the diagnosis (if you have a confirmed diagnosis), how the diagnosis was made (for example, biopsy, blood test, ultrasound, MRI or CT scans), and to provide the results of these tests. The referral specialist can set up an appointment with the Sloan-Kettering doctor who is best for that specific kind of cancer.
It may be more convenient for you to see a Sloan-Kettering physician at one of our outpatient sites in Basking Ridge, NJ, or in Sleepy Hollow, Rockville Centre or Commack, NY. Not all types of cancers are treated there, and some procedures are not offered, but if you prefer one of these locations, the referral specialist can guide you in how to make that appointment.
